Welcome to Mulvey Elementary School Health Office!

 

New York State Laws pertaining to:

NEW ENTRANTS: All new entrants must provide:

  • Proof of immunizations from doctor:

    4-5 DTap, 3 Hepatitis B, 3-4 Polio, 2 chicken pox, and 2 measles, mumps, rubella (MMR)

  • Physical and Dental form- Must be dated for current year. Expired forms will not be accepted

     

    New York State Laws pertaining to:

    PHYSICALS: Physicals are to be done by your child’s Health Care Provider.  An examination may be accepted if it is dated not more than twelve months prior to the start of the school year in which the examination is required.

  • All new entrants
  • Kindergarten, 1st, 3rd, 5th grades

     

    New York State Laws pertaining to:

    SCREENINGS: During the school year, I will provide the following:

  • Kindergarten- vision and hearing
  • 1st grade- vision and hearing
  • 3rd grade- vision and hearing
  • 5th grade- vision, hearing, and scoliosis (girls)
  • 2nd and 4th provided by Primary Doctor.

    Parents are only notified if there are any findings that would cause concern or need medical follow-up. A referral form will be given to the student to take home. Parent is expected to take student to their Health Care Provider. The referral form must be completed by doctor and returned to Mulvey Health Office ASAP.

     

    Requirements and information for school year:

     

  • Emergency Health Office Cards- Cards will be sent home the first day of school. Cards must be returned to school the next day. Include numbers where you can be reached. If your phone number changes during the school year, it is the responsibility of the parent/guardian to inform health office.

 

  • MEDICATIONS: Prescriptions, Over the counter medications
    • For safety reasons, Medications that your child needs to take during school MUST be brought to school by an adult
    • Medications must be in original packaging and must not be expired
    • A doctor’s order is needed
    • Parents written consent is needed

      All medications are kept in the Health Office.

      If your child has a documented life-threating allergy, I must have an Allergy Action Plan signed by Doctor and written permission from parent along with medication needed. Your child will be excluded from any activity that may put him/her at risk for exposure to allergen including field trips and outdoor recess. If your child no longer is considered at risk, I must have written confirmation signed by your child’s Doctor.  

       

  • END OF YEAR MEDICATION PICK UP- Medications need to be picked up by parent, guardian, or an adult at end of year. I will send a reminder letter home with information on dates and times for pick up. STUDENTS WILL NOT BE GIVEN MEDICATIONS TO BRING HOME. Any medication not picked up will be disposed of according to the NYS Department of Environmental recommendations.

 

  • SCHOOL BREAKFAST AND LUNCH- Mulvey Elementary School provides a monthly menu. Please review the menu with your child. If your child does not like the food choices, please ensure he/she brings in a packed meal from home.
  • SICK DAY RULES- Any child with a fever, rash, vomiting, or lice MUST be picked up from school and cannot go home on the school bus or walk home with out an adult. If child is sent home from school the child must be free of fever, rash, vomiting, and illness for 24 hours before returning to school. If your child is absent from school, due to illness for 3 or more days, a Doctor’s note is required and expected when child returns to school. Please report to Health Office with required Doctor’s clearance on the morning your child is able to return to school.

    Any child with a contagious condition, broken/fracture bone, or needs to use crutches in school needs a note from their Heath Care Provider upon or before returning to school.

    PARENTS AND GUARDIANS: Deciding to keep your child home from school is no easy decision. Use this guide to help determine if your child should take a sick day from school, as well as what to expect.

     

    Fever

    Children should not be in school if they have a temperature of 100?F or greater. Please keep your child home until fever is gone for 24 hours without the use of fever reducing medication. During flu season, children with a fever and other flu-like symptoms may be asked to stay home longer. Fever may be a sign of a more serious problem. Seek medical advice for fever lasting longer than 3 days.

    Vomiting and Diarrhea

    Children should not attend school if they have vomiting and/or diarrhea. Please keep your child home until 24 hours after the last time they vomited and/or had diarrhea. If vomiting and/or diarrhea continues and your child is unable to keep liquids down, you should seek medical advice.

    Skin Rash

    Skin rashes or irritation develop for many different reasons. Some are highly contagious and can cause serious problems. It is important to seek medical advice to determine the cause. In some cases, children may need to stay at home until the rash is gone. You are required to bring in a doctor’s note before returning to school.

    Cough-Cold-Sore Throat

    Children with productive coughing, sneezing, headaches, body aches, earache and/or sore throat may not be well enough to participate in school. Seek medical advice immediately if your child experiences difficulty breathing, wheezing or sudden high fever, chills and body aches.

    Eye Irritation

    Eye drainage, crusting, pain or redness may be a sign of infection and should be evaluated. If your child is diagnosed with an infection requiring treatment, he or she must stay home for 24 hours after treatment begins. You are required to bring in a doctor’s note before returning to school.

     

    *Doctor’s note is needed for all absences of 3 or more days.  Please call attendance when your child will not be in school 348-5053.

     

  • LICE- Your child will be sent home for lice treatment. Please report to Health Office the following day before the start of school with your child for clearance with:
    • Empty box of medication used for lice treatment

     

  • OUTDOOR RECESS- The students will go out for recess each day (weather permitting).Make sure your child comes to school dressed appropriately according to weather conditions.Please follow the Central Islip School District policy regarding our dress code. If your child comes to school inappropriately dressed (according top policy), the student’s parent is then responsible to drop off a change of clothes.

 

  • HEALTH OFFICE VISITS- Please check your child’s backpack daily for any information I may send home regarding your child’s health and well being. Parents are expected to respond in a timely manner for:

 

  • Health referrals sent home
    • Doctor’s note needed
    • Request for physical, dental, and medication concerns
    • Asthma Action Plan
    • Allergy Action Plan

       

      If you have any health issues concerns please feel free to contact me. Appointments are recommended. Required forms can be located online or picked up at the Health Office during school hours. You can also send a note to school with your child requesting any form.

      INFORMACIÓN DE LA ENFERMERA 

      Bienvenidos a la Oficina de Salud de la Escuela Elemental Mulvey

      De acuerdo con las Leyes de Salud del Estado De New York:

      TODOS LOS NUEVOS ALUMNOS DEBERAN PRESENTAR:

      • Prueba del Médico de las inmunizaciones siguientes:

        4-5 DTap, 3 de Hepatitis B, 3-4 de Polio, 2 de Varicela, 2 de Sarampión, Paperas, Rubiola

      • Prueba de Examen físico y examen dental con fecha del año vigente. No aceptaremos formularios con fecha caducadas.

        De acuerdo con las Leyes de Salud del Estado de New York con referencia a:

      • EXÁMEN FÍSICOS: Aceptamos exámenes físicos del médico del niño.

        Deben presentar examen físico:

      • Todos los alumnos nuevos
      • Alumnos de Kindergarten, 1er, 3er, 5to grados

        De acuerdo con las Leyes de Salud de Estado de New York con referencia a:

        PRUEBAS: Durante el año escolar, estaré proveyendo las siguientes pruebas o exámenes:

      • 1er. Grado – Examen de vista y auditivo
      • 3er. Grado – Examen vista y auditivo
      • 5to. Grado -Examen de vista, auditivo
      • 2do.Grado y 4to. Grados – Examen provisto por el Medico
      • Enviaremos información a los padres solamente si encontramos alguna anomalía que amerite atención médica. Proveeremos a su niño una nota de referencia al médico. Se espera que el Padre o Tutor lleve el niño al médico.La nota de referencia médica deberá ser llenada por el médico y devuelta a la Oficina de Salud de la Escuela Mulvey lo más pronto posible.

        Requisitos e Información para el nuevo año escolar:

  • Tarjetas de Información en caso de Emergencia – Estas tarjetas serán enviados a casa el primer día de clases. Deberán ser llenada y devueltas a la escuela el día siguiente. Incluya los números de teléfono donde se le pueda contactar.Si usted cambia números de teléfono durante el año escolar, es su responsabilidad, como padre/tutor, informarnos del cambio.

MEDICINAS: Recetas médicas, o medicinas sin receta medicas

  • Por razones de seguridad, todas las medicinas que su niño necesite tomar durante el día DEBERAN ser traídas por un adulto a la escuela.
  • Todas las medicinas deberán estar en su propia caja original y deberán tener fechas vigentes.
  • Se necesita una orden del médico.
  • Se necesita un permiso por escrito del padre/madre/tutor.

Todas las medicinas se mantienen en la oficina de la enfermera.

 

Si su niño padece de alergias que pongan su vida en peligro, yo debo tener un Plan de Acción firmada por el médico y un permiso por escrito de los padres/tutor junto con la medicina que necesite. Su niño será excluido de las actividades físicas que lo puedan poner en peligro, o expuesto a los alérgenos incluyendo excursiones, viajes al campo o recreo afuera del aula. Si su niño ya no está considerado con riesgos, yo debo de obtener una verificación por escrito firmada por el médico.

 

  • MEDICINAS AL FINAL DEL AÑO ESCOLAR: Todas las medicinas deberán ser recogidas por los padres/tutor o un adulto al final del año escolar. Yo enviare notas a sus casas para informarle la fecha y la hora para venir a recoger las medicinas. NO LE ENTREGAREMOS A NINGUN NIÑO MEDICINAS PARA LLEVARLAS A CASA.

    Cualquier medicina que no sea recogida de la escuela será desechada de acuerdo con las recomendaciones del Departamento del Salud Ambiental.

  • DESAYUNOS Y ALMUERZOS EN LA ESCUELA – La Escuela Elemental Mulvey provee un menú mensualmente. Por favor revise el menú con su niño. Si a su niño no le gusta las comidas, por favor asegúrese que él/ella traigan su comida de casa.
  • REGLAS DE DIAS DE ENFERMEDAD – Si su niño tiene fiebre, erupciones de la piel, o piojos, él/ella tendrá que irse a casa. No podrá tomar el bus y tendrá que ir a casa acompañado por un adulto. Si su niño fue enviado a casa con fiebre, vomito, erupciones de la piel u otra enfermedad, el puede regresar a clases 24 horas después de la ultima fiebre. Si su niño está ausente de la escuela debido a enfermedad por 3 días o más, requerimos una nota de su doctor cuando él regrese a clases. Favor de llamar a la Oficina de Salud por la mañana cuando su niño regresa a clases para avisarnos que puede regresar a clases con la aprobación del médico.

    Cualquier niño que tenga una enfermedad contagiosa, fractura de huesos o si necesita muletas en la escuela, necesita una nota de su médico antes o al momento de regresar a la escuela.  

     

  • PIOJOS – Si su niño tiene piojos, será enviado a casa para obtener tratamiento. Favor de reportarse junto con su niño a la Oficina de Salud al día siguiente del tratamiento y antes del comienzo de clases para obtener autorización para entrar a clases.
  • Favor de traer la caja de la medicina usada para tratamiento para los piojos.

 

  • RECREOS – Los estudiantes saldrán a recreo todos los días (si el tiempo lo permite). Asegúrese que su niño viene a la escuela vestido apropiadamente de acuerdo con las condiciones del tiempo. Por favor siga las reglas del código de vestuario del Distrito Escolar de Central Islip. Si su niño se presenta a clases vestido inapropiadamente (de acuerdo con las reglas de vestido) los padres del estudiante serán responsables de traer ropa adecuada para cambiarse.

     

  • VISITAS A LA OFICINA DE SALUD – Favor de checar la mochila de su niño diariamente por alguna información que yo tenga que enviar a casa con referencia a la salud y bienestar de su niño.

Se espera que los padres respondan tan pronto posible enviando los siguientes documentos:

 

  • Referidos del médico
  • Notas del médico
  • Formularios para examen médico, dental y autorización para medicinas
  • Plan de acción para el Asma
  • Plan de acción para alergias

Le recomiendo que me llame al 631-348-5056 o haga una cita si usted tiene alguna condición médica o preguntas. Todos los formularios requeridos pueden ser obtenidos en el Internet o en la Oficina de Salud durante las horas de escuela. Usted puede enviarme una nota con su niño pidiendo cualquier formulario.
